Who We Are

Founded in 2007, our vision is to transform end-of-life care in the communities we serve. We believe all hospice eligible patients and those that love them deserve the best care. We serve all patients and their loved ones the way they desire to be served. Our team consists of medical directors, registered nurses, social workers, chaplains, hospice aides, bereavement coordinators, and volunteers.

What We Do

Our mission is simple: we serve all hospice-eligible patients and their loved ones the way they desire to be served. We offer extended support through our social workers, spiritual care team, and volunteers. Heart of Hospice covers all medications and supplies that are related to the hospice diagnosis. In addition to these services, we provide thirteen months of bereavement care and support to those who are transitioning through the loss of a loved one.
