Our continuum of integrated home-based care services ensures patients and their families have the support they need to address current and future health needs. Partnering with Compassus brings peace of mind to patients, caregivers, and health care providers. Our team members work collaboratively to manage patient conditions and meet goals of care alongside referring physicians, health systems, long-term care partners, and family members. Our value is in the compassion and expertise of our teams, which act as an extension of patients’ existing care teams. We believe everyone who interacts with us should experience our individualized Care for who I am philosophy, from the care services we provide to patients and families to how we interact with our partners and one another.

What We Do

Our mission is simple: we serve all hospice-eligible patients and their loved ones the way they desire to be served. We offer extended support through our social workers, spiritual care team, and volunteers. Compassus covers all medications and supplies that are related to the hospice diagnosis. In addition to these services, we provide thirteen months of bereavement care and support to those who are transitioning through the loss of a loved one.

• Bereavement Support: Prepare and send mailings, Provide direct contact with the bereaved through home visits and/or phone o Calls with the purpose of companionship and support, Assist with the details of Memorial Services and Grief Support Groups, Be a Grief Support Group Leader depending on education and training, Be a Bereavement Ambassador to nursing homes and other facilities.
